Instructions to fill the Examination Form
i)	Description of subjects and their corresponding numbers for Licentiate Examination as well as a list of centres at which the examinations are generallly held are given below. Refer to them while completeing the entries in item Nos. 3 & 6
ii)	Payment only by Cash/ Money Oredr or Demand Draft in favour of local institute will be accepted. Postal Order and Cheques will not be accepted.
iii)	Candidates for Fellowship Examination should give their Associateship Diploma numbers inforce and the receipt number in column 8 (c) on the entry form.

Pattern Of Questions

In respect of CIS and Inspectors Examination,generally,the papers will be of objective type.For Licentiate Examination,three questions will be of objective nature and the rest will be subjective.As Far as other examinations are concerned, namely, Associateship & Fellowship, the questions will be subjective, but problems and case studies will also be involved.

Hints For The Examinees
The following hints will be useful to the examination candidates in answering questions under examination conditions.
- - -	A week before the examination commences,check your time-table. Check the exact date and time of each subject. Make sure of the examination venue so that you know how to get there.
ON THE NIGHT BEFORE THE EXAMINATION
- - - -	Avoid working too late at night. Check again the examination location,date and time etc. Try to get a good nights sleep,on the night before the examination. Ensure that you take pens,a sharpened pencil,calculator etc. with you.
ON THE MORNING OF THE EXAMINATION
- - - - -	Take light breakfast. Start well in time so that you can travel and reach comfortably to the examination hall. Get into the room well in time to find the seat allotted to you. Be comfortable and confident. The invigilators will issue the examination papers just before starting time but you should not commerce writing until told.
ON RECEIPT OF THE PAPER
- - -	First look at the paper carefully. Check that it is for the correct subject. Read the whole paper.It is not waste of time.
READ THE INSTRUCTIONS
-	Check which questions,if any,are compulsory and look at the problems (often these carry more marks)
-	Check how many questions must be attempted.
-	Answer only the required number of questions.
-	Allocate your time to each question and its sub-sections.
-	Commence with the relatively easy questions first.
-	Failure to follow the examiners requirements causes loss of marks even though you may do a good piece of work.
-	If them questions ask for a "definition", do not give a detailed discourse.A definition is formal and concise statement of the meaning of a word or phrase.similarly questions like "discuss","elaborate" etc. may be asked.These must be answered to meet the phrases used.
-	Organise your thoughts before you write anything.
-	Put your ideas in writing cogently.On Completion,check that you have dealt with all parts on the question and its sub-sections.
-	Commence each answer on a fresh page.
-	Do ensure that there is some space at the end of each question.This is useful when you read through the paper in the final quarter hour.
-	Avoid padding or repeating the answer to fill up pages.
-	On completion,read through everything.You might discover silly errors or omission or repetition which can be corrected.
-	Be legible,neat and tidy.This can make all the differences and the examiners will be impressed.
-	Remember,there is no substitute for hardwork.
-	Procrastination is known as "thief of time".Avoid this.
-	Success in examination is normally guaranteed to those who are organized, disciplined and regular in their studyhabits.

Change in Branch/Subject/Centre
A candidate will not be allowed to change the centre/subject(s) / branch indicated by him in the entry form submitted for examination. Error in the submission of entry form will not entitle the candidate to claim either refund or adjustment of entry fees paid by him/her. The fee paid shall be forfeited in such circumstances. The candidates must therefore fill in the entry forms with the utmost accuracy. No letter or communication of any type should be sent along with the entry form.
